Programs determined within this unit are:
- Assisted Living
- PACE
- Hospice Residential
- Chronic Care Medicaid in a Hospital
- Permanent Nursing Home Placement
- Short Term Rehab 29-days or less
- Temporary Stay - up to 6-months for rehabilitation
For a determination to be completed, all programs require filing an application and submitting program specific verification. Eligibility is based on income and resources. All applicants must submit a Supplement A application. If you are not already receiving Medicaid through the local Department of Social Services, all applicants must fill out an Access NY application as well.
The 2026 Income and Resource Limits for Chronic Care Medicaid are as follows:
Resource for a Single Individual: $32,396.00
Resources for a Community Spouse: $74,820.00
- Please note you may qualify for a higher resource limit as a spousal case based on your resources and circumstances. Please contact the Chronic Care Department for additional information.
Income level for 1 person: $1,800.00/monthly (Short-Term stay, Temporary stay and Assisted Living)
- Please note if you are permanently placed in a Skilled Nursing Facility or in Hospice Residential, you are only eligible to keep $50.00/monthly in income.
Income level for 2 people: $2,433.00/monthly (Community Medicaid)
Community Spouse Income Allowance: $4,066.50 (Skilled Nursing, PACE and Hospice Residential)
